School Choice Scholarship Vouchers and SGO Grants

School Choice Scholarships - New Applicants

In order to apply for the School Choice Scholarship Program (commonly referred to as the "Voucher" program) at St. Anthony School, a family must meet the following requirements:
  • Be a legal resident of Indiana
  • Student(s) must have been enrolled in an Indiana public school (grade 2-11) for at least 2 full, consecutive semesters.  (See SGO information below for Kdg. and Gr. 1)
  • Student(s) must be between the ages of 5 and 22 no later than Aug. 1 of the school year.
  • Have  income that falls within the eligibility chart, which is based on the Free/Reduced Lunch income requirements. 
  • Be admitted to St. Anthony School based upon our admittance guidelines.

School Registraton and School Choice Scholarship Vouchers
School Choice Scholarships (Vouchers) will be issued on a"first come-first served basis," based on eligibility, for classes with openings and a lottery process for classes where the number of applicants exceed the number of available seats.  Students will need to be enrolled in St. Anthony School prior to completing an online state application at the school with the Principal or another school representative. 

Steps for school and voucher enrollment:

  1. Complete a new student enrollment form.
  2. Complete a family enrollment form.
  3. Submit enrollment forms along with birth certificate, immunizations, baptismal certificate, ISTEP scores and last cumulative report card for each student to school office.
  4. Enrollment documents will be reviewed for admission eligibility.
  5. A date and time for an interview and placement testing will be arranged.
  6. Enrollment in our FACTS system at https://www.factstuitionaid.com/FACTS/gaapp for income verification is required.
  7. Voucher enrollments with the state will be finalized with a parent meeting at a later date.

School Choice Scholarships - Renewal Applicants

All School Choice Scholarship Recipients from 2011-12 need to complete the following steps:

  1. Complete a family enrollment form.
  2. Enroll in FACTS system at https://www.factstuitionaid.com/FACTS/gaapp.  This is a requirement to determine income eligibility for the 2012-13 school year.
  3. Confirm income falls within the eligibility chart guidelines.
  4. Voucher enrollments with the state will be finalized with a parent meeting at a later date

SGO Grants 

SGOs are a pre-cursor to the School Choice Scholarship Grants and are only available to families who will be enrolling a Kdg. student or a 1st Grader (who attended Kdg. in a public school).  A $300 credit can be given for tuition and St. Anthony will work with you to determine appropriate financial assistance. Each student will be eligible for the Indiana School Choice Scholarship Program for grades 2-12, should they meet eligibility requirements.  Families must be registered in our school and complete tuition assistance enrollment through the FACTS system.  Please see the FACTS Tuition Assistance sheet below for more information on enrollment.
